Adolescents and Young Adults with Special Needs

"What are the options for my son after he graduates from High School?"

"I would like to pursue some more education after High School. What are my options?"

" Where can my daughter go to find a job that will be a good fit for her needs?"

"My son is 20 and he wants more independence. Are there housing options available to him?"

When a child with special needs moves into adolescence, it is important to maintain or find the support services needed to ensure a successful transition into adulthood. Wisconsin First Step can help families and professionals find available services for adolescents and young adults. Parent Specialists can provide information on mental health resources, special education, learning disabilities, transition services, employment, centers for independent living and more.

Wisconsin has a statewide transition website that can be helpful to families who have an adolescent transitioning into adulthood. Wisconsin Statewide Transition Initiative

Resources to ease the transition to adult health care can be found at Building a Medical Home Partnership: A Wisconsin Toolkit
